Pengaruh layanan konseling dalam menurunkan tingkat depresi pasca stroke di Rumah Sakit Bethesda Yogyakarta

Latar Belakang : Kasus stroke di Rumah Sakit Bethesda Yogyakarta, merupakan urutan pertama terbanyak di seluruh rumah sakit di Daerah Istimewa Yogyakarta pada tahun 2004. Jumlah kasus stroke pada tahun 2004 sebanyak 950 kasus stroke. Sedangkan jumlah depresi akibat stroke dari bulan September dan Nopember 2005 sebanyak 74%, yang terdiri dari depresi ringan 43 %, depresi sedang 26 %, dan depresi sedang 5%. Depresi pasca stroke dapat dikurangi, salah satunya adalah menggunakan Konseling pasca stroke. Tujuan Penelitian adalah untuk mengetahui perbedaan penurunan depresi pasca stroke. Metode : Menggunakan kuasi eksperimen dengan membandingkan 2 kelompok yaitu kelompok eksperimen dan kelompok kontrol. Rancangan penelitian menggunakan pre-test dan post-test. Sampel yang diambil dengan cara accidental sampling selama bulan Februari, Maret, April 2007, sebanyak 34 kasus depresi pasca stroke untuk kelompok eksperimen dan 34 kasus depresi pasca stroke untuk kelompok kontrol. Intervensi menggunakan konseling pada kelompok eksperimen sedangkan kelompok kontrol tidak menggunakan konseling. Alat pengumpul data menggunakan alat ukur depresi Beck (BDI : The Beck Depression Inventory), dengan 21 pertanyaan. Pengumpulan data setiap pasien dilakukan pre-test, dilakukan konseling 2-3 kali, pada hari ke 6 dilakukan post-test, sedangkan untuk kelompok kontrol tidak dilakukan konseling. Analisis data menggunakan Independent t-test, sedangkan Paired sample t-test digunakan untuk menguji pengaruh layanan konseling dalam menurunkan depresi pada penilaian yang kedua (post test). Semua uji statistik dilakukan dengan tingkat kepercayaan 95% (alfa : 0,05) Hasil : Intervensi menggunakan konseling. Hasil analisis terhadap data penurunan depresi pasca stroke, diperoleh thitung sebesar= 11,781 dengan p (sig.) sebesar= 0,000. Ternyata p (sig.) lebih kecil dari taraf signifikansi yang ditentukan yaitu 5% atau 0,05; maka dapat dinyatakan bahwa ada perbedaan yang bermakna penurunan depresi pasca stroke di RS Bethesda Yogyakarta setelah perlakuan menggunakan konseling. Kesimpulan : Intervensi konseling pada pasien depresi pasca stoke, dapat menurunkan tingkat depresi pasca stroke.

Background: The number of stroke cases in Bethesda Hospital Yogyakarta was in the top rank among other hospitals in Yogyakarta Special Region in 2004. The total case number was 950 cases. Meanwhile the number of depression during September until November 2005 was 74% consisted of minor depression 43%, medium depression 26%, and major depression 5%. Post-stroke depression can be reduced; e.g. by counseling methods which was entitled as post-stroke counseling therapy. Method: Using research by comparing 2 groups, they were the experimental group and the controlled group. The plan of research is using pre-test and post-test. Samples were taken by having accidental sampling during February until April 2007. There were 34 cases of poststroke depression in the experimental group and 34 cases of post-stroke depression in the controlled group. Intervention was made by giving counseling only to the experimental group and not to the controlled group. Data collecting aid used was Beck Depression Inventory (BDI) which has 21 questions. The order of patients’ data collection for the experimental group were doing the pre-test, giving 2-3 times counseling, and on the 6th day doing the post test and for the controlled group were same but there was no counseling intervention. Data analysis was done by having independent t-test. On the other hand, paired sample t-test is used to test the impact of having counseling service in reducing depression on the post test. All statistics test were done with 95% of reliability level (Alfa: 0,05) Result: Intervention through individual counseling therapy. The analysis resulted in the decreasing of post-stroke depression was 11, 781 with p (sig.) = 0,000. It turned that p (sig.) was smaller than the required significance level, which is 5% or 0,05; therefore it can be said that there was a significant difference on the decreasing of post-stroke depression in Bethesda Hospital Yogyakarta after the research after counseling. Conclusion: Counseling intervention for the post-stroke depressed patients could reduce the post-stroke depression.
