Health Financing of District Health Office in Timor Leste

Introduction: As a result of the newly independent country and still in the implicit learning process of in the area of health, there are many shortcomings necessary to overcome, especially those in the financing management of public health. The implementation of the financing system must accommodate all of the existing problems. Method: The study is a descriptive and qualitative one and aims at describing the real condition and collecting actual fact of the financial planning of the public health at district level using purposive sampling method. The authors determine some districts, which are Bobonaro, Liquisa, Dilli, Lospalos, Same and Ermera districts and the subjects are 67 individuals consisting of the Head of the District Health Agencies, the Coordinator of District Programs, the Logistic Treasurer and the Public Health Director. Study Results: The results of the study show that the financial planning at district level has been conducted using bottom-up approach, but the planning of the health service activities has been made without any budgetary nomination of costs. The fund allocation system at the district level is implemented following the policy made by the Head of the Public Health Agencies because it is in accordance with the policy of the Central Government that has not explicitly determined the exact allocation for each of the working units of the programs and a proposal must be submitted and approved for the existing needs. Conclusion: The fund allocated by the central government has not adequately been accommodated for the working program of public health service. The allocating mechanism of the fund for the program has not follow the existing guideline and the distribution planning of the fund to the districts is centralistic in nature and the public health officers in remote areas have not got any access to the fund allocation.
